DKFZ CTO Core Dataset Survey

 

Welcome to the DKFZ Clinical Trial Office Core Dataset survey!

 

Dear Participants,

 

Thank you for taking the time to complete this survey.

 

The aim of the survey is to define a core dataset for clinical trials that ensures that the patient data documented includes as many data points as possible that are necessary for translational researchers.

 

The survey is voluntary and will be evaluated anonymously. The results will be communicated via the same channels as the invitation to the survey.

 

It takes about 15-30 minutes to complete the survey.

This survey is anonymous.

The record of your survey responses does not contain any identifying information about you, unless a specific survey question explicitly asked for it.

If you used an identifying token to access this survey, please rest assured that this token will not be stored together with your responses. It is managed in a separate database and will only be updated to indicate whether you did (or did not) complete this survey. There is no way of matching identification tokens with survey responses.


 

What happens with data I enter to pause and resume the survey later?

In case you enter a name and email address to resume the survey later, this information is only used by the system in order to send your personalized link to load the unfinished survey and is not known to the survey evaluators.

What happens with your data and how is it protected?

Your data will be used only for the research aim stated above. The anonymously collected data will be stored and evaluated at the DKFZ in Germany (Heidelberg). A transfer to a third party will not take place. We only reserve the option to publish the aggregated results.

Which data protection rights do you have?

The legal foundation of the data processing is your consent according to art. 6 § 1a of the General Data Protection Regulation (GDPR). Responsible in the sense of the (GDPR) is the DKFZ. You have the right to withdraw your consent at any time without the specification of reasons by clicking on "Exit and clear survey“ (top right corner of each page of the survey). In this case, all information you have entered so far will be deleted.

After submission of the survey a withdrawal of the consent is not possible anymore since the individual survey responses cannot be related to personal information. For this reason, you also cannot receive the information of data stored at DKFZ afterwards or request deletion, correction or limitation of this data. In case of concerns regarding the data processing and compliance with data protection you can contact the data protection officer of the DKFZ directly:
